Bleed bleed bleed. Nose bleed left and right. Blood come out from my mouth. Stop it can?

Since young this problem can't get away. Do housechore half way the bleed seems like water tap running down to the floor. Every where BLOOD BLOOD BLOOD. Rest on bed for 30mins still bleed. Go and take a bath as all my shirt and arm have blood BLEED AGAIN! Feel so dizzy.!

Sometimes even I am in train, or laughing with my friends.. Blood from my nose will auto drip down, and spilt out from my mouth. WHAT IS HAPPENING!!
